W2 Request Form

PLEASE READ FIRST

W2's for the 2022 were mailed out on the evening of January 27, 2023.

If you have not received your annual W2 notice in the mail by February 5, 2023 it may mean that the address we have on file is outdated and you will need to submit a request using this form.

If you need to request a reprinted copy of your W2 or if you have moved within the last year, please use this form to request a new W2. We recommend updating all mailing addresses no later than December 31 as our accounting team will be processing all annual W2 notices starting in January. W2’s are expected to be completed and mailed out to the address on file no later than January 31 (it may take 2-3 business days to reach your address).
