Mileage Reimbursement
Did you travel somewhere while working? Pickup a transfer from another store? Submit a mileage reimbursement. Please submit all reimbursements on the same day that travel occurs.

Submit Paid Time Off
Using sick time or taking vacation? Please submit your sick time usages on the same day that it is taken. Please submit all other paid time the day before your leave begins. Time Punch Adjustment
Forgot to clock in from a break?
Please submit all time punch corrections within 24 hours of initial mistake. All adjustments must be submitted no later that 12PM on Sunday. This is to ensure all corrections are properly marked before closing of pay periods.
Project Time Log
If you were assigned a project, training, or administrative task to complete and it was completed when you were not clocked in, fill out this form to log your time to your next paycheck.
